What Is Direct Primary Care?
Direct Primary Care, or DPC, is monthly membership that provides unlimited access to a doctor without copays, deductibles and without billing or using insurance.
Plum Health members get to call, text, email or visit their provider anytime to get primary care services like physicals, preventive care, urgent care, managing chronic diseases, and mental healthcare, without worry about hidden fees.
Meet Your Detroit Physicians:
Dr. Paul Thomas, CEO & Medical Director, is a board-certified family medicine physician and the founder of Plum Health Direct Primary Care, a growing network of multiple clinics across Michigan serving thousands of patients. Plum Health partners with large businesses, municipalities, and community organizations to provide affordable, accessible, and relationship-based primary care.
Founded in Detroit’s historic Corktown neighborhood, Plum Health DPC was the first Direct Primary Care practice in Detroit and Wayne County. Dr. Thomas’s mission is to transform how healthcare is delivered—shifting the focus from rushed, transactional visits to personalized, preventive, and comprehensive care that empowers patients to live healthier, more fulfilling lives.
Under his leadership, Plum Health has expanded into multiple communities while maintaining the same commitment to affordability and accessibility. His work has been featured on PBS, WDIV-TV Channel 4, WXYZ Channel 7, Crain’s Detroit Business, and CBS Radio, and he has been a featured speaker at TEDxDetroit.
Dr. Thomas is a graduate of the Wayne State University School of Medicine, where he now serves as a Clinical Assistant Professor, mentoring the next generation of physicians. He is also the author of three books, including Direct Primary Care: The Cure for Our Broken Healthcare System and Startup DPC: How to Start and Grow Your Direct Primary Care Practice.
Through his clinical work, teaching, writing, and advocacy, Dr. Thomas continues to champion a patient-first approach to medicine, building a healthcare model that serves both individuals and communities.
Dr. Raquel Orlich, DO is board certified in Family Medicine and Obesity Medicine. She has been a part of Plum Health since 2019. She is passionate about quality patient-centered care and establishing strong doctor-patient relationships. Her clinical interests include women’s health and preventative health. She specializes in medical weight loss and provides a comprehensive approach to help them reach their goals through nutrition, exercise, lifestyle intervention, and medical therapies. Lastly, Dr. Orlich is certified in administering Botox for cosmetic and therapeutic use and platelet rich plasma (PRP) for various skin issues and hair growth concerns. She is a graduate of Michigan State University College of Osteopathic Medicine and completed her residency training at Ascension Macomb-Oakland Hospital.
Dr. Kayla Hinton is a board-certified family medicine physician with a strong interest in integrative medicine—bridging the gap between conventional and traditional approaches to care, with a focus on patient education and empowerment. Her clinical interests include osteopathic manipulative treatment (OMT), environmental health, pediatrics, and preventive care for patients of all ages.
She earned her medical degree from Michigan State University College of Osteopathic Medicine and completed her residency training at Henry Ford Hospital in Warren. As part of her practice, Dr. Hinton offers OMT sessions and has taught this skill at the national level.
Outside of work, Dr. Hinton enjoys spending time with her family, reading, traveling, and learning languages. She is currently conversational in Spanish and studying other languages such as Italian.
Dr. Donovan Ogboh, DO is a board-certified family medicine physician with a dedicated interest in preventive care, sports medicine, pediatric care, women’s health, and the ongoing management of chronic conditions. He believes in delivering individualized, patient-focused care by taking the time to learn each person’s unique story, lifestyle, and health goals. Dr. Ogboh collaborates closely with his patients to create personalized, evidence-based treatment plans that support their overall well-being.
In addition to his primary care expertise, Dr. Ogboh is certified in Botox offering aesthetic treatments as part of a holistic approach to overall wellness and confidence.

No, we don't bill or use health insurance at Plum Health DPC. We focus on delivering excellent health care services, focusing on you as an individual. We have 1 hour appointments so that we can really get to know who you are and understand your health goals, and help you meet those health goals.

The monthly membership covers excellent primary care by providing open access to our services. Feel free to visit us in our office, call us when you need us, text us, or email us. You can schedule your annual wellness exam, receive basic office procedures, and optimize your health while under our care. Our members will also have access to at-cost laboratory testing, at-cost imaging services, as well as whole-sale medications. See a list of our services here.

No, we see our patients on time so there is typically little to no wait. We also guarantee same-day or next-day appointments during the week. So, when you're a member, you can have the peace of mind that you'll be seen when you need it.

First, we use a wide variety of tools to help you with any diagnosis. As family medicine specialists, we are trained in a broad and diverse methods of caring for any diagnosis. The biggest tool that we have is time - time to read and understand about your diagnosis and do our best work for you.
We also have a tool called RubiconMD, where we can take your symptoms and story, and any diagnostic work (labs, imaging) that we perform in our office, and send it to any specialist that we need to employ to help us answer your clinical question at no cost to you. We can send your case to psychiatry, dermatology, cardiology, orthopedics, etc... there's over 200 board-certified specialists on the platform that can help us with your diagnosis or treatment.
Next, if you have PPO insurance, we can refer you to any doctor in your PPO network. If you are uninsured, we have a list of specialist doctors like cardiologists, ophthalmologists, or dermatologists that offer cash pricing for their medical services.
Finally, if you have an HMO insurance plan, we could attempt to make a referral on your behalf to a specialist in your HMO network. About half of the time, our referrals via the HMO get rejected because the HMO administrators are restrictive. In that case, you'd have to see the primary care physician on the back of your insurance card to get a referral to the specialist of your choice. We can provide a letter for you to take to your PCP of why we think that referral is necessary.
